The New Hampshire Amateur Hockey Association will provide grant assistance to member associations to support special ADM related activities where the expense of equipment or service is an excessive burden to the association. This application must be completed, with supporting information, and submitted to the President of NHAHA. All submitted documentation will be reviewed and a response will be issued to the stated association contact within 45 days of the submission date (please plan accordingly).

Please note that the maximum grant award will be $1,000.00 per association per NHAHA fiscal year (September 1st to August 31st).

INSTRUCTIONS:

Please complete the Grant Form with sufficient detail to enable the Grant Committee to properly evaluate your request.  If a particular section does not apply, please do not leave it blank, but indicate N/A.  You may attach other relevant documentation that you would like the Grant Committee to consider, or submit a formal proposal.
